En Route Care
En Route Care provides targeted medical interventions during patient movement from point of injury to definitive care. It combines hemorrhage control, airway and ventilation management, damage-control resuscitation, physiologic monitoring, and telemedicine-enabled decision support to stabilize polytrauma, TBI, shock, and other combat-related conditions in austere or prolonged-evacuation environments.
Technical Challenges
Physiologic deterioration (hypovolemia, shock, respiratory compromise, secondary brain injury) during extended transport
Limited diagnostic and laboratory capability in moving platforms (no continuous point-of-care labs, constrained imaging)
Environment constraints: vibration, noise, limited space, power, and temperature control during aeromedical or ground transport
Variability in provider training, protocols, and manpower during austere, distributed, or coalition operations
Emerging Opportunities
Portable, durable blood products and cold-chain-independent transfusion solutions for forward and en route use
Integrated multi-parameter monitoring with artifact-resistant sensors for motion and vibration
Compact, field-ready decision-support/telemedicine tools that fuse physiology, diagnostics, and guideline-based recommendations
Hemorrhage-control and resuscitation devices optimized for confined transport platforms and prolonged field care
Current and Emerging Technologies in En Route Care
Portable blood and resuscitation products
Lyophilized plasma, whole blood collection and warm-storage systems, and low-volume hemostatic resuscitation kits that enable transfusion and damage-control resuscitation in transport.
Rugged continuous physiologic monitoring
Artifact-tolerant ECG, noninvasive hemodynamics, continuous capnography, regional cerebral oximetry, and wireless sensor fusion for reliable trend detection during movement.
Point-of-care diagnostics and imaging
Rapid labs (lactate, coagulation, hemoglobin), portable ultrasound with automated protocols, and microfluidic assays that guide en route triage and resuscitation decisions.
Compact hemorrhage-control and endovascular tools
Transport-optimized tourniquets, hemostatic dressings, rugged negative-pressure suction, and small-form REBOA-compatible devices for damage-control interventions during transport.
Telemedicine, AI decision support, and connectivity
Low-bandwidth video, cloud/edge analytics, and guideline-driven algorithms that augment en route teams, enable remote specialists, and support prolonged-field-care decisions.
Importance to Military Medicine
Reduce mortality and morbidity
Timely hemorrhage control, airway management, and resuscitation during transport are proven to decrease preventable deaths from combat trauma and polytrauma.
Enable prolonged and distributed care
When evacuation is delayed, en route capabilities extend lifesaving interventions and mitigate secondary injury in austere or contested settings.
Preserve operational readiness
Rapid stabilization and improved outcomes sustain force strength, reduce long-term disability, and speed return-to-duty where possible.
Optimize scarce resources
Efficient en route interventions decrease downstream demands on surgical and critical-care assets at definitive care facilities.
