Abstract
A dual mode automatic device for delivery of remote ischemic preconditioning treatment includes a cuff and a controller configured to deliver a first regular treatment protocol or a second extended treatment protocol. Switching to an extended treatment protocol may be done by an operator for selected groups of patients such as elderly or patients with diabetes. Automatic determination of the best mode is also disclosed. Optimal delivery of remote ischemic preconditioning treatment in this individualized patient-specific manner provides strong ischemic protection to a greater number of patients.
Core Innovation
A remote ischemic preconditioning therapy is delivered by inflating and deflating a cuff positioned about a limb of a subject according to a predefined first treatment protocol, where the first protocol includes a first predetermined number of treatment cycles. Each treatment cycle includes inflating the cuff to a cuff pressure at or above a limb occlusion pressure, maintaining the cuff pressure at or above the limb occlusion pressure for a first occlusion interval, deflating the cuff to a deflated state, and maintaining the cuff in the deflated state for a first release interval.
Before initiation of the first treatment protocol and as well as during or shortly after completion of the first treatment protocol, the subject is identified as having a preexisting condition necessitating applying a predefined second treatment protocol. The second treatment protocol provides an ischemic preconditioning stimulus of increased intensity relative to the first protocol.
Delivery of the remote ischemic preconditioning therapy is switched to the second treatment protocol when the subject has the preexisting condition, such that the total number of treatment cycles or a total treatment time exceeds that of the first treatment protocol. In this way, therapy intensity is increased by switching protocols based on preexisting conditions.
Claims Coverage
The provided partial set includes one independent claim. The claim centers on switching from a first predefined remote ischemic preconditioning protocol to a second predefined protocol of increased intensity based on identification of a preexisting condition, where the switched therapy results in exceeding the first protocol in total treatment cycles or total treatment time.
First cuff protocol with predetermined cycles and occlusion/release intervals
Inflating and deflating a cuff positioned about a limb according to a predefined first treatment protocol, where the first protocol includes a first predetermined number of treatment cycles; each cycle inflates the cuff to a cuff pressure at or above a limb occlusion pressure, maintains the pressure for a first occlusion interval, deflates the cuff to a deflated state, and maintains the deflated state for a first release interval.
Identify preexisting condition necessitating increased-intensity second protocol
Identifying the subject as having a preexisting condition necessitating applying a predefined second treatment protocol, wherein the preexisting condition is identified at any time before initiation of the first treatment protocol as well as during or shortly after completion of the first treatment protocol.
Switch to second increased-intensity protocol exceeding first protocol total cycles or time
Switching delivery of the remote ischemic preconditioning therapy to the second treatment protocol based on the preexisting condition, such that a total number of treatment cycles or a total treatment time exceeding that of the first treatment protocol is obtained.
The claim covers a remote ischemic preconditioning delivery that begins with a predefined first protocol using cuff inflation/deflation cycles, identifies a preexisting condition requiring a predefined second protocol, and switches to that second protocol so that total cycles and/or total treatment time exceed those of the first protocol.
Stated Advantages
Provides an ischemic preconditioning stimulus of increased intensity.
Results in a total number of treatment cycles or a total treatment time exceeding that of the first treatment protocol for subjects with the preexisting condition.
Documented Applications
Delivery of remote ischemic preconditioning therapy to a subject based on identification of a preexisting condition.
