Abstract
Provided herein are subcutaneous formulations of ketamine that are useful for treating a variety of disease and disorders. The subcutaneous ketamine formulations provided herein reduce injection site irritation and pain.
Core Innovation
The invention relates to pharmaceutical compositions that include an active pharmaceutical compound having a cationic ionized base and a complexing agent comprising a substituted cyclodextrin with a plurality of acidic functional groups. At least one acidic functional group functions as a counter-anion to the cationic ionized base of the active pharmaceutical compound. The compositions address solution behavior by comparing a first solution containing water versus a second solution containing a sodium salt of the complexing agent, with the acidic functional groups serving as counter-anions to sodium in the second solution.
In the disclosed compositions, osmolality is used as a differentiating parameter between formulations. The first solution consisting of the active pharmaceutical compound at a first concentration, the complexing agent at a second concentration, and water is lower in osmolality than the second solution that includes equivalent concentrations but further includes sodium cations via the sodium salt of the complexing agent. The acid groups in the second solution are counter-anions to sodium, thereby increasing the osmolality of the second solution relative to the first solution.
Another aspect specifies a pharmaceutical composition in which sodium cations are removed from a sodium salt of a substituted cyclodextrin so that at least one acidic functional group is a counter-anion to an ionized nitrogen cation of the active pharmaceutical compound. A corresponding osmolality comparison is defined between a first solution made from equivalent amounts of the active pharmaceutical compound, the sodium salt-based substituted cyclodextrin component, and water, and a second solution with equivalent amounts but in which the sodium cations have not been removed.
A further aspect is an aqueous pharmaceutical composition formed by removing at least one sodium from a sodium salt of a substituted cyclodextrin having acidic functional groups and replacing the removed sodium with an active pharmaceutical compound having a cationic nitrogen. This removal-replacement is described as reducing osmolality of a composition formed by combining the active pharmaceutical compound and the sodium salt, compared to an equivalent composition formed without removal of the at least one sodium.
Claims Coverage
The independent claims, three in total, center on substituted cyclodextrins with acidic functional groups and osmolality relationships determined by whether sodium is present as a sodium salt or removed and replaced or counter-ion exchanged with an active pharmaceutical compound cation.
Complexing agent with acidic functional groups as counter-anion to cationic ionized base
A pharmaceutical composition comprising an active pharmaceutical compound comprising a cationic ionized base and a complexing agent comprising a substituted cyclodextrin with a plurality of acidic functional groups, where at least one acidic functional group is a counter-anion to the cationic ionized base of the active pharmaceutical compound.
Lower-osmolality water solution versus higher-osmolality sodium-salt solution based on counter-anions
A formulation relationship where a first solution consisting of the active pharmaceutical compound at a first concentration, the complexing agent at a second concentration, and water has lower osmolality than a second solution consisting of the active pharmaceutical compound at the first concentration, a sodium salt of the complexing agent at the second concentration, and water, wherein acidic functional groups in the second solution are counter-anions to sodium, thereby increasing osmolality relative to the first solution.
Substituted cyclodextrin sodium removal enabling counter-anion to ionized nitrogen cation
A pharmaceutical composition comprising an active pharmaceutical compound having an ionized nitrogen cation and a sodium salt of a substituted cyclodextrin with a plurality of acidic functional groups, from which sodium cations have been removed such that at least one acidic functional group is a counter-anion to the ionized nitrogen cation of the active pharmaceutical compound.
Lower-osmolality solution after sodium cation removal compared to equivalent sodium-retained solution
A formulation relationship where a first solution consisting of the active pharmaceutical compound, the sodium salt component, and water has lower osmolality than a second solution with equivalent amounts but in which sodium cations have not been removed from the substituted cyclodextrin.
Removing sodium from cyclodextrin salt and replacing with cationic nitrogen active pharmaceutical compound to reduce osmolality
An aqueous pharmaceutical composition formed by removing at least one sodium of a sodium salt of a substituted cyclodextrin having a plurality of acidic functional groups and replacing the removed sodium with an active pharmaceutical compound having a cationic nitrogen, wherein removal reduces osmolality of a first composition relative to a second composition formed with equivalent concentrations in which the at least one sodium has not been removed from the salt.
Across the independent claims, the inventive focus is the use of substituted cyclodextrins bearing acidic functional groups as counter-anions, combined with explicit osmolality comparisons between formulations that differ by whether sodium cations are present as a sodium salt or removed and replaced or counter-ion exchanged with the active pharmaceutical compound cation.
Stated Advantages
Increased osmolality in the formulation where the acidic functional groups act as counter-anions to sodium relative to the corresponding water-based first solution.
Lower osmolality in the formulation where sodium cations are removed from the substituted cyclodextrin and acidic groups counter-anion to the ionized nitrogen cation.
Reduction of osmolality resulting from removing at least one sodium from a sodium salt of the substituted cyclodextrin and replacing it with an active pharmaceutical compound having a cationic nitrogen.
Documented Applications
Subcutaneous injection for pain and psychiatric/cognitive/neurological disorders, including major depressive disorder, post-traumatic stress disorder, Huntington’s disease, and Parkinson’s disease.
Clinical-study protocol outlines and claimed tolerability or improved irritation outcomes associated with the described formulations.
Other explicitly mentioned therapeutic contexts include Alzheimer’s disease, spinal cord trauma, cancer pain, acute post-operative pain, and sterile abscess, with related irritation, erythema, and itching references.
