Abstract

Various systems, devices, and methods are disclosed herein for treating acute myocardial infarction (AMI) patients using a heart pump controlled in a manner that maximizes mechanical unloading of the left ventricle in the presence of cardiovascular instability and minimizes myocardial oxygen consumption (MVO2) and consequentially infarct size to prevent the development of subsequent heart failure. In a closed feedback system, the system can include a sensor configured to generate an output used to measure or calculate a left ventricular systolic pressure (LSVP) within the left ventricle of a heart and a controller coupled to a heart pump. The controller can be configured to measure or calculate the LVSP based on the output of the sensor and to control an operation of the heart pump to maximize mechanical unloading of the left ventricle based on the measured or calculated LVSP.

Core Innovation

The disclosed invention provides a cardiac support system for an acute myocardial infarction (AMI) patient in which a heart pump includes a first portion configured to be positioned within a left ventricle and a second portion coupled to the first portion and configured to extend through an aortic valve into an aorta when positioned. A first sensor positioned within the first portion generates an output used to measure or calculate a left ventricular systolic pressure (LVSP) within the left ventricle, and a controller measures or calculates the LVSP based on the sensor output.

The controller uses the measured or calculated LVSP to control pump operation so the system mechanically unloads the left ventricle based on the LVSP. The heart pump has a pump speed and a flow rate, and the controller is configured to control the pump speed or the flow rate to maintain the LVSP within the left ventricle at a target reference pressure.

The target reference pressure is set to reduce a pressure-volume area (PVA) of the left ventricle by about 90% to about 97%. The disclosed control approach further describes reduction of myocardial oxygen consumption (MVO2) within specified quantitative ranges, together with a closed feedback control system, an adaptive control framework, and an LVSP-PVA recruitment framework for selecting safe target LVSP levels.

Claims Coverage

The provided excerpt includes three independent claims: a system, a method, and a non-transitory computer readable storage medium. Across these independent claims, the inventive theme is LVSP-based control of a heart pump to maintain LVSP at a target reference pressure set to reduce left-ventricle PVA by about 90% to about 97%.

Cardiac support system for mechanically unloading based on LVSP

a heart pump comprising a first portion configured to be positioned within a left ventricle of a patient, and a second portion coupled to the first portion and configured to extend through an aortic valve of the patient and into an aorta of the patient when the first portion is positioned within the left ventricle of the patient; a first sensor positioned within the first portion of the heart pump and configured to generate an output used to measure or calculate a left ventricular systolic pressure (LVSP) within the left ventricle of the patient; and a controller coupled to the heart pump and configured to measure or calculate the LVSP based on the output of the first sensor and to control an operation of the heart pump to mechanically unload the left ventricle of the patient based on the measured or calculated LVSP.

Maintaining LVSP at a target reference pressure while controlling pump speed or flow rate

wherein the heart pump has a pump speed and a flow rate, and the controller is configured to control the pump speed or the flow rate of the heart pump, such that the LVSP within the left ventricle is maintained at a target reference pressure.

Target reference pressure set to reduce left-ventricle PVA by about 90% to about 97%

wherein the controller is configured to set the target reference pressure to reduce a pressure-volume area (PVA) of the left ventricle by about 90% to about 97%.

AMI treatment method using LVSP measurement or calculation and pump control

providing an AMI patient; positioning a first portion of a heart pump within a left ventricle of heart of the AMI patient; positioning a second portion of the heart pump through an aortic valve of the heart of the AMI patient and into an aorta of the heart of the AMI patient when the first portion of the heart pump is positioned within the left ventricle of the heart of the AMI patient, the second portion of the heart pump being coupled to the first portion of the heart pump; measuring or calculating a left ventricular systolic pressure (LVSP) within the left ventricle of the heart of the AMI patient using an output generated by at least one sensor positioned within the first portion of the heart pump; controlling an operation of the heart pump to mechanically unload the left ventricle of the heart of the AMI patient based on the measured or calculated LVSP to treat the AMI patient, wherein controlling the operation of the heart pump comprises controlling a pump speed or a flow rate of the heart pump, such that the LVSP within the left ventricle is maintained at a target reference pressure.

Setting target reference pressure to reduce left-ventricle PVA by about 90% to about 97%

setting the target reference pressure to reduce a pressure-volume area (PVA) of the left ventricle of the heart of the AMI patient by about 90% to about 97%.

Non-transitory computer readable medium for LVSP-based heart pump mechanical unloading

measure or calculate a left ventricular systolic pressure (LVSP) within a left ventricle of a patient based on an output of a sensor of a heart pump; and control an operation of the heart pump to mechanically unload the left ventricle of the patient based on the measured or calculated LVSP, wherein the heart pump comprises (a) a first portion configured to be positioned within the left ventricle of the patient and (b) a second portion coupled to the first portion and configured to extend through an aortic valve of the patient and into an aorta of the patient when the first portion is positioned within the left ventricle of the patient, wherein the sensor is positioned within the first portion of the heart pump, wherein controlling the operation of the heart pump comprises controlling a pump speed or a flow rate of the heart pump, such that the LVSP within the left ventricle is maintained at a target reference pressure.

Target reference pressure set to reduce left-ventricle PVA by about 90% to about 97% (computer-implemented)

wherein the target reference pressure is set to reduce a pressure-volume area (PVA) of the left ventricle by about 90% to about 97%.

Across the independent claims, the coverage centers on LVSP-sensed control of heart pump pump speed or flow rate to maintain LVSP at a target reference pressure, with the target set to reduce left-ventricle PVA by about 90% to about 97%. The independent claims define the structural placement of pump portions and sensors, the LVSP-based mechanical unloading control, and the quantified PVA-reduction target.

Stated Advantages

Reduce a pressure-volume area (PVA) of the left ventricle by about 90% to about 97%.

Reduce myocardial oxygen consumption (MVO2) in specified ranges.

Documented Applications
