Abstract
A device for tensioning a surgical thread comprising: an elongated body having a tip portion adapted to be arranged in contact with a knot of a surgical thread wound about a patient's bone and a maneuvering member connected to the elongated body by a screw and nut mechanism configured to adjust the distance between said elongated body and said maneuvering member, said maneuvering member having respective coupling portions for coupling opposite ends of the surgical thread wound and knotted about the bone. The opposite ends of the surgical thread reversibly constrainable to the respective coupling portions in a use configuration of the device. The distance between the elongated body and the maneuvering member can be increased along a movement axis to tension the opposite ends so as to tighten the surgical thread about the bone. The tip portion has a groove shaped to receive at least partially the knot.
Core Innovation
The invention provides a device for tensioning a surgical thread used for bone cerclage. An elongated body extends along a longitudinal axis and includes a tip portion adapted to contact a knot of a surgical thread wound about a patient's bone. A maneuvering member is connected to the elongated body and is arranged outside the patient's body, with coupling portions for coupling opposite ends of the surgical thread wound and knotted about the bone.
The elongated body is connected to the maneuvering member by a screw and nut mechanism configured to adjust the distance between the elongated body and the maneuvering member. The distance can be increased along a movement axis coinciding with the longitudinal axis to tension the opposite ends of the surgical thread and progressively tighten the surgical thread about the bone. The tip portion has a groove shaped to at least partially receive the knot and to define respective return portions for routing the opposite ends back toward the coupling portions.
The routing arrangement places the opposite ends of the surgical thread tangentially to the bone over a first stretch between the return portions inside the groove, and transversely to the bone along respective second stretches that have a greater length than the first stretch. A bone cerclage kit is also described that includes a soft biocompatible fabric thread tied into a knot and tightened by the device.
Claims Coverage
The relevant independent claim identified is directed to a complete bone-cerclage tensioning device having a screw-and-nut distance adjustment mechanism, a tip groove with return portions for knot contact and thread routing, and a maneuvering member outside the body that couples opposite thread ends. The main inventive features include a longitudinally aligned tip-to-maneuvering-member adjustment, knot-receiving groove geometry, and a two-part thread routing geometry with tangential and transverse stretches.
Elongated body with tip portion contacting the knot
An elongated body extending along a longitudinal axis with a tip portion adapted to contact a knot of a surgical thread wound about a patient's bone.
External maneuvering member coupling opposite thread ends
A maneuvering member connected to the elongated body, arranged outside the patient's body, with respective coupling portions for coupling opposite ends of the surgical thread wound and knotted about the bone, wherein the opposite ends are reversibly constrainable to the respective coupling portions in a use configuration of the device.
Screw and nut mechanism adjusting distance along the longitudinal axis
The elongated body is connected to the maneuvering member by means of a screw and nut mechanism configured to adjust the distance between the elongated body and the maneuvering member, wherein the distance can be increased along a movement axis coinciding with the longitudinal axis to tension the opposite ends and tighten the surgical thread about the bone.
Groove perpendicularly extending with return portions for routing
The tip portion has a groove shaped to at least partially receive the knot, the groove extending perpendicularly to the movement axis and opening on opposite sides of a sidewall to define respective return portions along which the opposite ends can be returned toward the coupling portions.
Tangential first stretch and longer transverse second stretch
The opposite ends of the surgical thread are arranged tangentially to the bone over a first stretch between the return portions inside the groove, and transversely to the bone along respective second stretches, the second stretches having a greater length than the first stretch, between the return portions and the coupling portions.
Overall, the independent claim centers on a device that tightens a bone-cerclage surgical thread using a screw-and-nut mechanism that increases a longitudinally aligned distance between an elongated tip portion contacting the knot and an external maneuvering member coupling opposite thread ends. The claim further defines knot contact and thread routing through a groove with return portions and a two-stretch routing geometry with a shorter tangential first stretch and a longer transverse second stretch.
Stated Advantages
Reduced risk of soft-tissue injury versus metal twisted-thread snag, as emphasized for the soft biocompatible fabric thread.
Enables cutting excess after tightening with scissors.
Documented Applications
Orthopedic bone cerclage where a surgical thread is wound about a fractured bone (bone cerclage tensioning about a patient's bone).
Use configuration in which the device tensions and progressively tightens a surgical thread knot by adjusting an external maneuvering member connected to an elongated tip portion.
Bone cerclage kit application including a soft biocompatible fabric thread tied into a knot and tightened by the device.
Embodiments that provide tension detection/indication during use via a window/scale and indicators related to spring force and/or stem insertion depth.
