Abstract
A system for measuring central venous pressure is provided comprising a device for measuring jugular venous pressure in communication with a patient inclination controller via a processing unit.
Core Innovation
The invention provides a system configured to measure central venous pressure in a patient by measuring jugular venous pressure using an optical device. The optical device includes at least one light source emitting light in the 400 nm to 1000 nm wavelength range and a plurality of adjacent photodetectors that receive the emitted light and transmit a current or voltage output signal.
Central venous pressure is determined by measuring the distance from the phlebostatic axis to the highest position on the jugular vein that yields the output signal. The highest position is based on the output signal transmitted by a photodetector adjacent to a photodetector that transmits no significant output signal, so that the system identifies the highest vein position using the photodetector outputs.
To enable detection of the highest position, the system includes a patient inclination controller in communication with a processing unit and a linear actuator. The patient back support is movable between a horizontal base position and a multitude of inclined positions, or between a vertical base position and a multitude of declined positions, and is adjusted when the processing unit activates the control circuit.
The processing unit receives and processes the output signal from each photodetector to calculate central venous pressure and activates the patient inclination controller so the back support reaches a position that permits the optical device to detect the highest position along the jugular vein. A waveform or output signal is produced for a display, and communication between the processing unit and the inclination controller is used to support the measurement.
Claims Coverage
The independent claim covers a non-invasive optical jugular venous pressure measurement system integrated with automatic patient back inclination control, with central venous pressure computed from a detected highest jugular vein position relative to the phlebostatic axis. The dependent claims refine specific optical/probe configurations and processing/display components.
Optical device for jugular venous pressure measurement
An optical device configured to measure jugular venous pressure, comprising at least one light source adapted to emit light in the 400 nm to 1000 nm wavelength range, and a plurality of adjacent photodetectors each adapted to receive light emitted by the at least one light source and transmit a current or voltage output signal.
Distance-based CVP determination using highest jugular vein position
Central venous pressure determined by measuring the distance from the phlebostatic axis to the highest position on the jugular vein to yield the output signal, wherein the highest position is based on the output signal transmitted by a photodetector adjacent to a photodetector that transmits no significant output signal.
Patient inclination controller with linear actuator and movable back support
A patient inclination controller comprising a control circuit in communication with the processing unit and a linear actuator, and a patient back support movable between a horizontal base position and a multitude of inclined positions by the linear actuator, or a vertical base position and a multitude of declined positions by the linear actuator.
Automatic back support adjustment based on photodetector output patterns
The processing unit activates the control circuit to activate the linear actuator to incline the patient back support from the horizontal base position when the output signal is transmitted by all of the plurality of photodetectors, or to decline the patient back support from the vertical base position when no significant output signal is transmitted by any of the plurality of photodetectors, to achieve a position of the back support that permits the optical device to detect the highest position along the jugular vein.
Adjacent photodetector arrangement over a constrained device length
A plurality of adjacent photodetectors positioned along a device length of about 1.5 to 10 cm.
Light-delivery/receiving probe for external tissue site near the jugular vein
At least one light-delivery probe guiding light from at least one light source to a patient’s external tissue site near the jugular vein and receiving reflected and/or transmitted light at that site using multiple adjacent photodetectors.
Embedded light source and photodetectors within a probe
At least one probe in which at least one light source and a plurality of adjacent photodetectors are embedded.
One-to-one mapping between multiple light sources and adjacent photodetectors
Multiple light sources where each light source emits light received by a corresponding photodetector among multiple adjacent photodetectors.
Processing unit with microprocessor and display unit
A processing unit that contains a microprocessor and a display unit.
Across the independent claim and dependent refinements, the core system integrates an optical device with adjacent photodetectors in the 400 nm to 1000 nm range, distance-based CVP determination using the highest jugular vein position identified by an output-to-no-output photodetector transition, and automatic patient back inclination control using a linear actuator driven based on whether all photodetectors transmit output or none transmit significant output. Dependent claims further specify photodetector arrangement, probe configurations, and microprocessor/display components.
