Abstract

The invention relates to improved methods, devices, and kits for identifying and implementing an appropriate treatment regimen for subjects suffering from hypertension.

Core Innovation

The invention provides a method for treating an individual patient having hypertension by administering a first line therapy antihypertensive drug selected from a group consisting of a diuretic drug, a vasodilator drug, and a beta blocker drug. The approach selects among these drug classes based on a genotype of the individual patient for fourteen specified single nucleotide polymorphisms, and the vasodilator drug is limited to an angiotensin II receptor antagonist (blocker) drug or an ACE inhibitor drug.

The method administers the first line therapy antihypertensive drug according to procedures a, b, c, d, e, f or g that link particular genotype conditions to selection of the first line drug class. Specific homozygous conditions at rs1159744 or rs2107614 select a diuretic drug procedure without administering the beta blocker drug and the vasodilator drug, while homozygous conditions at rs4961 or rs1529927 select thiazide or thiazide-like diuretic drug use. Specific homozygous conditions at rs3892097, rs1801253, rs1801252, rs1042714, and rs1042713 guide selection of the beta-blocker drug with genotype exclusion rules.

Additional genotype-linked procedures select an angiotensin II receptor blocker vasodilator drug when the genotype is homozygous for cytosine at rs12750834 or rs5186 and not homozygous for specified variants associated with other procedures. ACE inhibitor vasodilator drug selection is determined by homozygosity for a deletion in rs1799752 or cytosine at rs699 with exclusion of variants used for diuretic and beta-blocker linked procedures. A further procedure administers an amiloride potassium sparing diuretic drug when the genotype is homozygous for adenine at rs2228576 and not homozygous for specified variants, and a final procedure administers a loop diuretic drug when the patient does not meet homozygous conditions required for prior procedures.

Claims Coverage

The claims provide one independent claim covering genotype-guided selection and administration of a first line antihypertensive regimen using fourteen specified SNPs and multiple genotype-linked procedures.

Genotype-guided first line antihypertensive selection

A method for treating an individual patient having hypertension by administering a first line therapy antihypertensive drug selected from a group consisting of a diuretic drug, a vasodilator drug, and a beta blocker drug, wherein the diuretic drug is selected from the group consisting of a thiazide diuretic drug, a loop diuretic drug and an amiloride potassium sparing diuretic drug and the vasodilator drug is selected from the group consisting of an angiotensin II receptor antagonist (blocker) drug and an ACE inhibitor drug; determining the individual patient's genotype for each of fourteen specified single nucleotide polymorphisms and administering the first line therapy antihypertensive drug according to one of procedures a, b, c, d, e, f or g.

Fourteen specified SNP genotyping condition set

Determining the individual patient's genotype for each of fourteen single nucleotide polymorphisms: rs1159744; rs2107614; rs4961; rs1529927; rs3892097; rs1801253; rs1801252; rs12750834; rs5186; rs1799752; rs699; rs2228576; rs1042714; and rs1042713.

Genotype-linked drug-class procedures a–g

Administering the first line therapy antihypertensive drug according one of the following procedures a, b, c, d, e, f or g based on specified homozygous genotype conditions and exclusion of specified homozygous conditions, including selection of diuretic drug, beta-blocker drug, angiotensin II receptor blocker vasodilator drug, ACE inhibitor vasodilator drug, amiloride potassium sparing diuretic drug, or loop diuretic drug.

The claims tie a fourteen-SNP genotype determination to administration of a selected first-line antihypertensive drug class using genotype-conditional procedures with specified homozygous requirements and exclusions.

Stated Advantages

An improved blood pressure response compared with what would occur after administering a different antihypertensive drug.

Documented Applications

Genotype-guided selection of antihypertensive drug class therapy for an individual patient having hypertension, including linkage of specific SNP genotypes to differential blood pressure response and time to blood pressure control under diuretic, ACE inhibitor, angiotensin II receptor antagonist, and beta-blocker therapies.
